This Patient Safety Incident Response Plan sets out how ZEST Midwives Ltd (hereafter referred to as
ZEST), intends to respond to safety incidents over a period of 12 to 18 months. We will remain flexible
and consider the specific circumstances in which safety issues and events occurred and the needs of
those affected.

PSIRF Policy
This Patient Safety Incident Response Policy has been developed for ZEST Midwives Ltd (hereafter
referred to as ZEST). The glossary below defines key terms used throughout this policy to ensure
clarity, consistency and shared understanding across all colleagues, ZEST registrants and partners.
This policy supports the requirements of the Patient Safety Incident Response Framework (PSIRF) and
sets out ZEST’s approach to developing and maintaining effective systems and processes for
responding to safety events for the purpose of learning and improving patient safety.
